ABSTRACT

COMPREHENSIVE MANAGEMENT OF DEPRESSION BEYOND SSRIS: CURRENT PHARMACOLOGICAL AND NON-PHARMACOLOGICAL STRATEGIES

Anitha Sadula*, Usha Rani Peddaboina, Shaheen Begum, Pendyala Meghana, Mudiga Anuradha

Major depressive disorder (MDD) is a globally debilitating condition. While Selective Serotonin Reuptake Inhibitors (SSRIs) are the standard first-line pharmacological treatment, they are limited by delayed onset, adverse effects like emotional blunting, and inadequate efficacy, with up to half of adults failing to achieve remission on their first trial. This review evaluates comprehensive MDD management strategies beyond traditional SSRIs, synthesizing evidence on novel pharmacology, non-pharmacological interventions, and precision medicine. Advanced pharmacological strategies overcome SSRI limitations using novel mechanisms. Multimodal antidepressants like vortioxetine modulate multiple serotonin receptors to enhance efficacy, particularly for cognitive symptoms. Rapid-acting glutamatergic modulators, including esketamine and dextromethorphan-bupropion, provide swift relief for treatment-resistant depression. Furthermore, neurosteroids such as zuranolone and brexanolone offer targeted, rapid treatments specifically for postpartum depression. Beyond pharmacotherapy, comprehensive care relies heavily on non-pharmacological strategies. Psychotherapies like Cognitive Behavioral Therapy (CBT), combined with lifestyle interventions like exercise, significantly reduce depressive symptoms and prevent relapse. For severe or resistant cases, neuromodulation techniques such as repetitive transcranial magnetic stimulation (rTMS) and electroconvulsive therapy (ECT) demonstrate substantial efficacy. Additionally, precision medicine is transforming treatment pathways. Pharmacogenomic (PGx) testing utilizes individual genetic profiles-particularly CYP2D6 and CYP2C19 metabolic enzymes-to guide antidepressant selection and dosing, thereby improving remission rates and minimizing adverse events. Integrated collaborative care models further optimize these outcomes by pairing psychiatric expertise with primary care. Managing MDD requires a shift from trial-and-error SSRI monotherapy to a personalized, biopsychosocial model. Integrating novel multimodal agents, rapid-acting therapeutics, neuromodulation, behavioral interventions, and pharmacogenomics maximizes therapeutic success and patient well-being.
